   Transcript Release Form Information: Students requesting release of academic transcripts will be asked to pay a $5.00 fee. Download the PDF file, print out and complete the Transcript Release Form, and mail it to Northwest Iowa Community College, 603 West Park St., Sheldon, IA 51201. You can also fax the request with a credit card number and expiration date to 712-324-4136. If you have any questions regarding a transcript request, please call Tara at 1-800-352-4907 ext. 167. |
